Grant Amendment Requests 

To request a budget revision, payment schedule adjustment, or timeline extension, please complete the Grant Amendment Request Form. The form can be accessed by logging in to the Grantee Portal, navigating to Active Grants, selecting the relevant grant project, clicking Active Grant Forms, and selecting Add Form to begin a new amendment request.

Grant Payments 

All grant payments are tied to reporting. If your payment has been placed on hold due to underspending and you have since spent down your funds, please submit a Grant Amendment Request to request payment disbursement. 

Grantees must request capital payments when they are ready to make a purchase through the Grantee Portal.

Evaluation Resources


Evaluation Timeline

All grantees will be required to submit a report every six months during the project period. Further instructions, including the exact dates, will be shared closer to each reporting period. Technical assistance will be provided to support grantees throughout this process.

During active reporting periods, the Evaluation team will offer daily "office hours" where grantees can schedule a 15 or 30 minute block of time with a member of the Evaluation Team for questions about reporting or troubleshooting issues with the Secure Reporting Portal. Available time blocks can be viewed and scheduled at the link below. If you need to schedule a session outside of the available times, please email Evaluation@oneohiofoundation.com.

EVALUATION AND DATA COLLECTION WEBINAR

This webinar will cover important information about data collection and program evaluation.  Your organization can receive payments only after completing evaluation reports demonstrating appropriate program implementation have been submitted.  Your main contact and your reporting contact should attend this webinar.
